Valir PACE is a Program of All Inclusive Care for the Elderly (PACE), and is the future of healthcare. It utilizes a team-based care approach providing a personalized, comprehensive healthcare plan to the most at risk, those without access to quality healthcare, the under and uninsured. Individuals enrolled in our program have better health outcomes, less visits to the emergency room, while continuing to live in their own home, often times without ever having to go into a nursing home.

Transportation is provided to all of our participants to come into the adult day center for socialization, the medical clinic and hot meals during the week. We also arrange for transportation to specialty doctor appointments, as needed. Additionally, as field trips and off-site activities are identified and planned in the community, participants are transported to and from the PACE center.

The focus of every PACE organization is to help the elderly live in the community for as long as possible. To meet this goal, PACE organizations focus on preventive care. Although all people enrolled in PACE are eligible for nursing home care, only seven percent live in nursing homes.
