Valir PACE is a program that provides comprehensive health care, medication, therapy, transportation, social interaction at the PACE center, hot meals, and even adaptive equipment for their home to provide a safe environment.

PACE is a full spectrum plan designed to meet your healthcare needs and promote vital, independent lifestyle through onsite clinic, pharmacy, gym and social center… all conveniently located and designed with you in mind.
Services provided by Valir PACE include:

Comprehensive medical, dental and specialist care

Personalized preventive care and management of chronic conditions

Adult day program including therapeutic recreation

Diagnostic and laboratory tests

Prescription medications, medical supplies and equipment

On-call Nurses and Physicians 24 / 365 to address your urgent health concerns

If needed, emergent and urgent care, hospital care, and nursing home care are also covered.
Some services listed above require pre-authorization and are made available based on a determination of physical and psychosocial needs made jointly by your Interdisciplinary team and you.
Participants may be fully and personally liable for the costs of unauthorized or out-of-PACE program agreement services.

Home Care
The Valir PACE Home Health Team will come to your residence to assist with household responsibilities as well as evaluate if any additional equipment (ramps, shower seats, handrails, etc) are necessary.
Transportation
Our transport team provides door-to-door service to all medically necessary appointments at no cost. This includes transportation to your specialty doctors’ visits and the Valir PACE day center for medical services.
